$28+/hr + super + up to $15,900 in salary packaging + retail discounts

About the role

In this exciting role, you will be responsible for providing operational support to the Home Care Team. Working closely with the Service Manager, your role will be fundamental in building your team for success, now and into the future. Primarily overseeing the coordination and delivery of care in the community, you will lead a team that enables clients to live the best life they can at home.

In this varied role you will:

  • Organise, conduct and document the planning, provision and assessment of community services for low to medium level client care needs;
  • Ensure compliance with community care best practices, privacy and legal obligations;
  • Provide advocacy and support for clients and people living with dementia and/or behaviours of concern;
  • Supervise and mentor staff; assist to ensure adequate resourcing of equipment and staffing levels;
  • Coordinate stakeholders and external services to enhance care for clients;
  • Promote the service within the catchment area; identify opportunities for expansion.

About you

You will bring a strong leadership background from the care, health or service sectors. Always considering the rights of the client, you work to ensure the quality and safe care services are delivered. You are able to identify client requirements and re-align services in response to identified needs.

Your experience and qualifications for the role should include:

  • A certificate IV in Home and Community Care (or equivalent) or commitment to complete this within 12 months of commencing;
  • Completion of HLTHPS006 - Assist clients with medication, HLTAAP001 Recognise healthy body system and CHCCCS011 Meet personal support needs;
  • Demonstrated leadership experience in a similar role;
  • Possess current First Aid and CPR certificates;
  • A current driver's licence and a willingness to use your own comprehensively insured vehicle for work purposes;
  • Applied knowledge of current aged care legislation, funding mechanisms, government policies, and statutory regulations;
  • The possessor has the ability to obtain; a National Police Check, National Personal Insolvency Check, Influenza vaccination and COVID vaccinations.
