About the role

You will be excited to lead and support a team of home care workers and support staff that directly impacts the lives of clients living independently in their own homes.

Your day will be varied and will include:

• Supervise and mentor staff;
• Assist to ensure adequate resourcing of equipment and staffing levels;
• Case management; including planning, provision and documentation of care;
• Identify and respond to the changing physical and mental health needs of our clients;
• Provide direct care to clients including in a supervisory and supporting role to staff.

About you

You can identify customer requirements and re-align services in response to identified needs. You are an exceptional communicator, both verbally, and in written form and enjoy working in a team environment.

Your experience and qualifications for the role should include:

• A minimum Certificate III in Home & Community Care or equivalent;
• Must have or be willing to complete HLTHPS006 - Assist clients with medication and HLTAAP001 Recognise healthy body system;
• Demonstrated leadership experience in a similar role;
• Possess a current Senior First Aid Certificate with resuscitation techniques;
• A current driver's licence and a willingness to use your own comprehensively insured vehicle for work purposes (km's paid);
• Experience in case management and the ability to identify and respond to risk;
• The possessor has the ability to obtain; a National Police Check, National Personal Insolvency Check, Influenza vaccination and COVID vaccinations.
